Pet Pantry Eligibility Guidelines
These guidelines are set in order for us to help as many families as possible. We ask that these guidelines are followed in order for a household to receive assistance:
- All participants must fill out the Application to Receive Assistance
- Participants are asked to provide basic information about their pet (species, number, approximate weight,etc) to ensure appropriate distribution of supplies
- Valid identification card or driver’s license
- Proof of residency (utility bill, pay stub, lease agreement, mortgage statement, etc)
- Proof of number of pets (rabies certificate, veterinary bill, dog license certificate, vaccination certificate, etc)
